Alignment efforts can help agencies provide more efficient, effective, and accessible services for individuals and families. However, agencies often face significant challenges related to funding constraints, policy barriers, data silos, technology infrastructure, and service delivery models.

What is this Roadmap?​
This Roadmap provides a structured approach to figuring out where to start, assessing readiness, identifying barriers, providing resources, and implementing integration strategies. These recommendations and resources are informed by cross-sectional insights garnered throughout the ACCESS project and are centered on the customer’s desired future state of alignment.

Who is this Roadmap for?​
This guide is intended for people engaged in planning for or implementing data alignment strategies at the state level in health, human services, or workforce agencies. This may include:
